Wrightington, Wigan and Leigh Teaching Hospital NHS Foundation Trust website provides information about the community diabetes service for people with Type 2 diabetes. The service aims to provide high quality information and advice for newly diagnosed patients with a DESMOND programme (DESMOND is a structured education program for adults living with Type 2 Diabetes),insulin initiation and titration and ongoing review.
The service is provided from clinical locations across the district, nursing and residential homes and home visits where appropriate.
