With regards to the recently published NHS 10 Year Plan and the creation of Neighbourhood teams to deliver prevention and care in the community, I would like to know the following information please:

1. How many Neighbourhood teams are being created within your jurisdiction/area?
2. What will each team be called?
3. Who is running the new Neighbourhood teams?
4. Which GP Network will hold the contract?
5. Who is the Chairperson of the relevant network running the Neighbourhood team?

NHS Greater Manchester does not hold the information requested as these matters have not yet been determined. It is expected that this information should be available by April 2026.
