FOI 2025/1318

I’m writing to request copies of the Excel spreadsheets referenced in your FOI responses:

• FOI 2025/1272 – “A list of all PCNs in your ICB and the Clinical Director and PCN manager in charge of that PCN.”
• FOI 2025/1280 – “Names and contact email addresses of Clinical Directors, PCN Managers …”

Your website notes that an Excel sheet was sent to the requesters and that we can request a copy by email. Please could you send the original files (ideally in .xlsx), or if easier a consolidated spreadsheet covering all Greater Manchester PCNs with the following fields where held:
PCN name; locality/borough; federation (if held); PCN manager name; PCN manager email; postal address for the PCN/federation/host organisation; Clinical Director name and email.

If any fields are not held centrally, please provide what you do hold. I’m happy to accept .csv if that’s quicker.
