Under the Freedom of Information Act 2000, we request the following information regarding your organisation’s commissioning and provision of Structured Diabetes Education (SDE) for people living with Type 1 and Type 2 Diabetes.

Additionally, we seek information about commissioning and provision of services or education for individuals with gestational diabetes.

As a current provider of SDE to several ICBs across the NHS in England, we are seeking to understand the current delivery landscape. Please provide answers to the following:
1. Who is the lead or main contact responsible for commissioning SDE for Type 1 Diabetes within your ICB? Please include name, job title, and contact email if available.
2. Who is the lead or main contact responsible for commissioning SDE for Type 2 Diabetes? Please include name, job title, and contact email if available.
3. Please confirm the name(s) of the provider(s) currently delivering SDE for both Type 1 and Type 2 Diabetes and the specific course(s) they provide (e.g., DESMOND, DAFNE, or others).
4. How many people were referred for Type 2 Diabetes Structured Education (SDE) during the most recent full financial year (April 2024 – March 2025)?
5. Of those referred for Type 2 Diabetes SDE, how many completed the full course?
6. How many individuals are currently on a waiting list for Type 2 Diabetes SDE?
7. What is the current average waiting time from referral to first education session for Type 2 Diabetes SDE?
8. What formats of structured education are offered (e.g., face-to-face, online live sessions, digital self-guided)?
9. Does your ICB provide any education or support for people diagnosed with gestational diabetes? If so, please include the name(s), format(s), and provider(s) if available.
10. Who is the lead contact for commissioning or overseeing gestational diabetes care and support (e.g. education, monitoring)? Please include name, job title, and contact email if available.

NHS Greater Manchester consists of the following 10 Greater Manchester Localities: Bolton, Bury, Heywood, Middleton and Rochdale, Manchester, Oldham, Salford, Stockport, Tameside, Trafford, and Wigan (former Clinical Commissioning Groups), who currently all have their own commissioning arrangements in place.

Oldham – NCA are commissioned to deliver face to face SDE for both DAFNE and DESMOND, as well as support referral to digital offers of the same.

Salford – NCA, T1D – DAFNE, T2D – XPERT, plus additional sessions have been developed to meet patient needs such as Carb Counting etc.
Stockport – Stockport NHS Foundation Trust is the provider for delivery of both type 1 & type 2 education. Courses delivered are X-PERT for type 2 diabetes (delivered online and face to face) and SOCCER (Stockport Optimising Carbohydrate Counting Education and Results) which is a locally designed bespoke programme delivered face to face. In addition to structured education, the Expert Patient Programme, which is open to all with Long-Term Conditions, is routinely accessed/attended by those with Diabetes. The My Way Diabetes platform (online) is also available to Greater Manchester residents who prefer self-paced/guided online access to education modules via the app.

Tameside – For patients recently diagnosed with type 1 diabetes, they provide face-to-face and virtual training through DAFNE, and for those recently diagnosed with type 2, through DESMOND

Stockport – There are 289 on the wait list for X-PERT however, 275 of these have been sent an invite letter to join courses starting in September or November with the provider awaiting the patient to opt into these.

Stockport – Wait To Be Seen/start course (June 2025) for X-PERT and expert patient programme = 10.7 weeks. Patients opt into the service and select the courses and venue which best suits their needs which may mean longer waits if they would like particular venues or timings.

Bury – F2F – DESMOND/Diabetes Essentials/DAFNE, online – DAFNE, digital self-guided – MyWay Diabetes.
HMR – Formats include face to face / group settings and online resources.
Manchester – Face-to-face, online live sessions, digital self-guided.
Oldham – F2F, digital/ self-guided.
Salford – In person and online face to face sessions alongside the NHS GM commissioned digital offer – My Way Diabetes and NHSE Healthy Living online support offer.
Stockport – X-PERT is delivered both online and face to face and SOCCER face to face. In addition to this NHS Greater Manchester commission the My Way Diabetes platform for online access of self-guided structured education modules via a patient app.
Tameside – The provider delivers face to face and digital online live sessions.
Trafford – Face to Face.
Wigan – For the WWL service digital self-guided and face to face plus however the diabetes myway delivers and the national one is self-guided digital.

HMR – A gestational diabetes service is commissioned as part of the NCA block contract (specialist diabetes service).
Manchester – Gestational Diabetes – MFT – Saint Mary’s hospital Antenatal Diabetes Services – Saint Mary’s Hospital
Oldham – Gestational diabetes support is via the acute provision in the NCA. Support is consultant led, and nurse supported. The details would need to be requested from the provider.
Salford – My Gestational Diabetes online course provides by My Way Diabetes; NCA deliver a monthly pre pregnancy clinic.
Stockport – Those with gestational diabetes are under the care of diabetes specialist midwives at Stockport Foundation Trust where group education or 1-1 support is offered.
Tameside – Yes, SDE includes Gestational diabetes, please contact the provider for more information. In addition, patients who have a history of gestational diabetes can be referred to the National Diabetes Prevention Programme, which is delivered by REED Wellbeing.
Trafford – High level support through community midwife services, MFT provide specialist support through the diabetes pregnancy team.
Wigan – My Gestational Diabetes online course provided by Diabetes My Way and SE provided through the midwifery service at WWL.
