I am writing under the Freedom of Information Act 2000 to request information about the Lung Cancer Screening (previously Targeted Lung Health Check) programmes in your ICB.

1. Do you have a Lung Cancer Screening [LCS] (or formerly Targeted Lung Health Check [TLHC]) programme running in your ICB region?

2. If your answer to Question 1 is Yes, please answer the following questions for each active LCS (TLHC) service / contract within the region:

1. 1. Areas covered
   2. Commissioner of the service (i.e. name of the ICB, NHS Trust, Cancer Alliance etc. who is leading on the programme in the ICB)
   3. Name of the provider who delivers the service (e.g. name of the NHS Trust or external provider)
   4. Start date and end date of the contract
   5. Can the contract be extended beyond its end date, and if yes, for how long?
