I am writing under the Freedom of Information Act 2000 to request information regarding OT-led sensory or functional assessments for adult patients receiving NHS Continuing Healthcare (CHC).
Please provide the following information, ideally covering the most recent 12 months and any previous periods for which records are available:

1. Number of Patients Assessed:
a. How many adult CHC patients have received an OT-led sensory or functional assessment (including PM-2, equivalent tools, or carer-based questionnaires)?
b. Please specify for the 100 adult CHC patients currently under care in Manchester and also neighbouring ICB’s

2. Assessment Responsibility:
a. Confirm that OTs are the professionals conducting these assessments not just in Manchester.
b. If any external OT or commissioned provider is involved, please provide details of the organisation(s) and any service agreements.
c. Confirm whether these OTs have prior experience conducting such assessments.

3. Use of Carer Input:
a. Are carer-based questionnaires solely used to complete OT assessments?
b. Who else completes them (family, paid carers, LD nurses, etc.), and how is this information incorporated into care planning?

4. Outcomes / Effectiveness:
a. If available, please provide information on assessment outcomes or evidence of how they informed care planning or CHC decisions.
b. Please also confirm whether there is a formal appeal or dispute process in place if the findings or recommendations of an OT-led sensory assessment are contested.

5. Historical Data:
a. Please include any previous OT-led sensory assessments commissioned by the ICB or LD team for adult CHC patients, including approximate numbers and timeframes.
