I am writing to request the following information under the Freedom of Information Act 2000.
Please provide details of all Primary Care APMS contracts currently active within your ICB. Specifically, for each APMS contract, I would like to know:
1. The practice name and ODS code
2. The original contract start date
3. The original contract end date
4. The initial contract term (e.g., 5 years, 10 years)
5. Any extension periods available under the contract (including their duration)
If available, I would prefer this information in Excel or CSV format.
