For your ICB, please provide a list of all ophthalmology service providers currently under contract, and those contracted within the past three years.
For each provider, please include (where held):
a. Organisation name and type (e.g. NHS Trust, Independent Sector, Optometry Practice)
b. Contracted service(s) (e.g. Community Ophthalmology, Tier 2, Cataract, Glaucoma, Triage)
c. Contract start and end/expiry dates
d. Contract status (active, expired, extended, etc.)
e. Service delivery locations (addresses, if available)
f. Contract or award reference (Find a Tender / Contracts Finder link, if applicable)
g. A generic contact email for the provider or service
