NHS Greater Manchester consists of the following 10 Greater Manchester Localities: Bolton, Bury, Heywood, Middleton and Rochdale (HMR), Manchester, Oldham, Salford, Stockport, Tameside, Trafford, and Wigan (former Clinical Commissioning Groups), who currently all have their own processes/systems for recording their Continuing Healthcare (CHC) activity.

Wigan – On occasions there may be a need to commission a care package off the Ethical Framework from domiciliary care providers on a spot purchase arrangement depending upon the patients assessed needs. These rates will be agreed on an individual basis.

In addition to this the overall allocation to the Clinical Commissioning Group (CCG) which existed prior to the creation of the Integrated Care Boards (ICBs) needs to be assessed alongside the organisational savings requirement to support delivery of the agreed financial position. All of these factors are put together and from this the uplift in any given year is derived, if one is not stipulated nationally. |

Non-Framework providers are dealt with on an individual basis. For Direct PHB’s again the uplift % is signed off by the Senior leadership Team (SLT). However, these are not applied automatically and are usually discussed as part of a 12-month review. |

For third party PHB’s the overall budget would be increased in line with national pay increase, and we would work with the employers to encourage them to increase carers hourly rates reflective of skillset and national % for pay increases whilst remaining within the allocated budget. |
| Trafford | The Local Authority undertake annually a ‘Fair Price for Care’ (FPfC) engagement with the care home market to assess views on the proposed uplift for domiciliary, residential and supported living packages of care. The Local Authority derive their proposed rate having regard for changes in National Insurance (NI) rates, real and national living wage increase and forecast average Consumer Price Index (CPI) for 2025/26. This engagement informs the final proposed uplift that is subsequently approved by the Local Authority Executive Council.
NHS GM has agreed to align the uplift on NHS CHC funded packages of care with the Local Authority approved inflationary uplift rate in which a care provider operates. This approach applies to placement with providers in GM. In exceptional circumstances an ICB locality may deviate from the Local Authority Fair Price for Care (FPfC) rate, in which case we would follow the rate agreed by the ICB locality for homes in that borough.
For care providers outside of GM, the CHC uplift rate is for local determination – for consistency this will be the Trafford Local Authority uplift rate. |
| Wigan | PHB Direct Payments /3rd Party
PA rates are calculated in line with agenda for change pay scales on an annual basis. Any uplifts are reviewed via Wigan Locality PHB Risk Panel.
PHB Notional Any packages that are on Wigan Council Ethical Rates will automatically increase in line with the Local Authority on 1st April. Any package that does not come under the Framework Rate will be required to provide a written request to Wigan Locality, each request will be reviewed via the Price Increase Panel and an uplift will be offered in line with Wigan Locality budget. |

All PHB’s have a support plan/costings in place which provides assurance that frontline staff receive uplifted rates provided by the ICB. A letter is sent to the PHB holder/responsible employer with the uplifted hourly rates and the PHB broker is informed who in general manages the payroll. |

Recently the locality has received Carecubed training to ensure that the cost of the care is being utilised in the most cost effective and efficient way including the pay rates for the cares. Carecubed is a software package that is a secure online care costing tool that supports open and transpartent negotiation of cost of care placements. |
