I am writing to request information under the Freedom of Information Act 2000.
Please provide the following details for all GP practices within the City of Manchester local authority area, current as at 13 November 2025. If you do not hold this information for all practices, please provide what you hold and indicate which practices I should contact directly.

1. Mental Health Practitioners / Nurses
Please provide a list of GP practices in Manchester that currently employ (directly or on a salaried/contracted basis) any mental health practitioner(s) or mental health nurse(s).
For each practice, please include:
• Practice name and full address
• Job title(s) of the mental health practitioner(s)/nurse(s) employed • Whether the practitioner is employed directly by the practice, employed by the Primary Care Network (PCN), or provided by a third party (and state the third party name if known) • Number of Whole Time Equivalent (WTE) mental health practitioners/nurses employed at the practice • If any of the practitioners are due to leave their role, please state:
• Their expected departure date, and
• The stated reason for leaving (e.g. resignation, retirement, contract ending, redeployment, etc.)

2. Focused Care Practitioners
Please provide a list of GP practices in Manchester that currently employ or host a Focused Care Practitioner, or a staff member working under the Focused Care model.
For each practice, please include:
• Practice name and full address
• Job title of the Focused Care Practitioner (or equivalent title used locally) • Whether they are employed by the practice, the PCN, Focused Care CIC, or another organisation (please specify) • Number of days per week (or WTE) the Focused Care Practitioner is based at the practice • If any of these staff are due to leave, please state:
• Their expected departure date, and
• The stated reason for leaving

3. Format and Coverage
Please provide the above information in a machine-readable format (CSV or Excel preferred).

Please find attached details of all GP practices within the City of Manchester local authority area, current as at 13 November 2025.

NHS GM does not hold details of staff employed by GP Practices and so is unable to provide details of Mental Health Practitioners / Nurse or Focused Care Practitioners.
