I would like to make an FOI request regarding your CHC care plans.
1. How many types of care plan do you have for CHC? (joint funded care plan separate to your Personal Health Budget ones etc)
2. How many pages long are they?
3. Would I be able to have a blank copy of any that you use?

1. A CHC care plan is compiled by the CHC Nurse Assessor based on the clinical needs of a patient who is receipt of CHC funding. This is in line with the national framework: National framework for NHS continuing healthcare and NHS-funded nursing care – GOV.UK
2. The 10 CHC Teams for NHS GM use an electronic patient record (EPR) system to record data. Therefore, the length of the documentation varies on the EPR system used, in addition to the individual complexity of the CHC package of care.
