I would like to request the following information under the Freedom of Information Act:

• The ICS’s aggregate score from the Digital Maturity Assessment for 2024
  • AND the individual Digital Maturity Assessment scores for each NHS trust in the ICS for 2024
• The ICS’s aggregate Digital Maturity Assessment score for 2025
  • AND the individual Digital Maturity Assessment scores for each NHS trust in the ICS for 2025

I’d like to request this information be formatted in a spreadsheet with columns for trusts, the 2024 score, and the 2025 score, with ICS aggregate scores for each year at the bottom of the respective 2024 and 2025 column, as set out in the template below.
