The purpose of my email is to identify some further information about future opportunities to support with complex care packages. We are keen to understand more about the volume of care packages covered currently, so that we can invest in the local provision to provide more care in the community and support complex care more where it’s needed.

Please can you help me identify any of the following info?

  1. What is the annual spend on complex care packages or alternatively, how many complex care packages or hours are covered currently monthly by providers?
  2. As it stands, we are a spot provider for complex care packages, I am hoping to confirm if there is an existing framework for complex care?
      1. If so, is there an opportunity to be considered for the framework?
      2. If there is no current framework in place, is there an intention to form one?
  1. How many providers currently work with you for continuing healthcare packages?

  1. NHS GM does not hold financial information about CHC and other Individual packages of care specifically to identify whether a case is considered as Complex Care or otherwise. Therefore, in response to question 1, NHS GM’s annual spend on CHC packages of care (excluding Funded Nursing Care) is approximately £384m, which will include cases which can be defined as Complex Care, as well as those which would not be categorised as such.
  2. The Greater Manchester Provider Framework – Supporting People to Live Well at Home is intended to be used to commission specialist provision to support people with complex care needs to live well at home.

This framework is for high-quality providers who can provide person-centred and outcome-focused care and support. Providers are asked to adopt a strength-based approach, supporting people to recognise and achieve their full potential, whilst also working towards increased levels of health and wellbeing.

The scope of this Framework includes:

  • Learning Disabilities
  • Neurodivergent
  • Complex Lives
  • Complex Mental Health
  • Complex Dementia
  • Individualised Commissioning Including Continuing Health Care (CHC)
  • Preparing for Adulthood

The framework covers the following types of service: Residential, Nursing, Supported living, Day opportunities, Homecare, Outreach, Respite and Short breaks, Supported Employment and Volunteering.

2a. The GM Framework – Supporting People to Live Well at Home, will open regularly so that new providers can be awarded a place on the framework, ensuring that the GM Framework remains responsive to changes in the provider market and to the changing needs of our population. The GM Framework will open to new providers later this year. The date the Framework will reopen is yet to be confirmed but we advise interested providers to register their details on The Chest  The Chest: North West Procurement Portal if not already registered, in order to receive notifications regarding procurement activity and to send your contact details, including your email address, to gmhscp.gmasctransformation@nhs.net so that an email notification can be sent to you when the procurement launch date is set.

  1. There are approximately 353 CHC providers used across NHS GM ICB (including both Nursing Homes and Domiciliary care Providers).
