Under the Freedom of Information Act 2000, I am requesting information regarding the primary care premises within your area.
Specifically, I am looking for data concerning GP surgeries that are currently operating, or have historically operated, in buildings originally constructed or designated for residential use. Please provide the following:
1. A list of all GP practice main and branch surgery addresses within the ICB’s jurisdiction.
a. For each address, please state the current Planning Use Class as held in your primary care estates database (e.g., Class E, or former Class D1).
b. Identification of “Residential Conversions”: Please identify which of these premises were converted from Use Class C3 (Dwellinghouses) to medical use.
c. Planning Reference Data: If held, please provide the local authority planning application reference number associated with the “Change of Use” for these specific sites.
d. Ownership Status: Please indicate whether these “converted” premises are GP-owned (under Notional Rent arrangements) or leased from a third-party provider/NHS Property Services.
If providing the full list exceeds the cost limit, please prioritize the list of surgeries identified as having a historical or current “residential” planning origin.
I would prefer to receive this information in an electronic format.
Please find attached list of all GP practice main and branch surgery addresses under the jurisdiction of NHS Greater Manchester (NHS GM).
NHS GM does not hold the detail requested in the sub-requests (a)-(e), therefore, it will be necessary for you to contact the practices directly for the information you seek.

Please note any individual’s names, contact numbers and notes have been removed from the spreadsheet as they are considered out of scope of the request. This information is exempt from disclosure under section 40 (2) of the Freedom of Information Act 2000 as it constitutes personal data where disclosure would be in breach of the Data Protection Act 2018.
As section 40 (2) is an absolute exemption there is no duty to consider disclosure in the public interest. They are set out in Part II of the Freedom of Information Act 2000
