1. The total number of patients who have been referred to Image UK, Charity Number: 1141833, by healthcare services part of Greater Manchester Integrated Care Partnership for each calendar year from 2020-2025.
2. A list of the names of the healthcare services part of Greater Manchester Integrated Care Partnership that have referred patients to Image UK, Charity Number: 1141833, in any calendar year from 2020-2025, with the number of referrals each have made.
3. A copy of any criteria used by practitioners’ part of Greater Manchester Integrated Care Partnership to verify the safety and quality of care provided by external services or organizations before patients are referred to them.
