Dear NHS, Greater Manchester Integrated Care Board, Please provide contract details on what ICB funding is provided to Viaduct Care CIC Stockport and a breakdown of which services are provided with these funding amounts.

NHS Greater Manchester ICB (NHS GM) commissions a range of services from Viaduct Care CIC as follows.

Viaduct Care CIC is the Stockport GP Federation. The services commissioned from Viaduct Care CIC are all borough wide services that are closely aligned to Primary Care Network (PCN) workings. This facilitates a model of primary care provision that promotes stability and resilience, primary care delivered at scale, continuity of care and high-quality outcomes and supports clinical leadership to the Stockport Locality.

Similarly, NHS GM commissions Viaduct Care to provide Primary Care Digital Facilitators.

The provider also supports all Stockport PCN’s and employs staff on behalf of PCN’s through the PCN Additional Roles Reimbursement programme. The programme is inherent within PCN Directed Enhanced Service as part of the GP national contract and provides funding to PCN’s so they can hire additional healthcare staff outside the traditional GP and nurse roles to support GP practices.

NHS GM also facilitates a WorkWell scheme, using funding from the Department of Work and Pensions. As part of this, Viaduct Care are paid to provide Work and Health Coaches to the scheme to assist local residents through their health challenges and into work.

Viaduct Care also host the Greater Manchester Primary Care Provider Board on behalf of GM. As part of this, funding is provided to Viaduct to pay the costs of clinicians supporting the Board, as we as the running costs of the Board in order to support primary care collaboration, development and improvement across Greater Manchester.
