I am writing to you under the Freedom of Information Act 2000 to request the following information from your organisation related to Community Diagnostic Centres. Please may you provide me with an answer to the following:

1. Please list the CDCs that are currently operational in your area.
   1. For each of these individual CDCs, please give:

      2. The name of the provider who operates it
      3. The full list of diagnostic services provided

1. Are there any plans to build future CDCs in your area?
