Under the Freedom of Information Act, I would like to request the following information regarding complex home care packages.

For clarity, I am defining ‘complex home care’ as a package of care involving over 100+ hours of care per week.

If you cannot break down data by number of care hours per package, then ‘complex home care’ should be defined as any package of care that requires significant nurse-led care involving specialised procedures and management that go beyond basic domiciliary home care assistance.

Failing that definition, please define “complex home care” as any home care package eligible for funding under the NHS Continuing Healthcare (CHC) funding pathway.

Please note I have requested a breakdown of the data by age group. If data cannot be broken down by requested age bands, then please provide total figures across all age groups, adults aged 18+ and children aged 0-17.

If data for the full year 2025/26 is not yet available, then please provide the latest yearly snapshot data for 2025/26.

Please kindly include responses on the attached spreadsheet.

  1. Please provide the name, telephone number and email address of the person responsible for coordinating / liaising discharge from hospital to community health and social care providers.

 

  1. Please provide the total number of patients discharged from hospital to CHC- funded complex home care in 2025/26, by NHS Trust or other hospital provider (please specify name and number of patients discharged by each hospital), broken down across:

  3. Adult patients aged 18-64 years
  4. Child patients aged 0-17 years

 

  1. Please provide the total number of bed days lost in 2025/26 due to lack of available CHC-funded complex home care packages, by NHS Trust or other hospital provider (please specify name and number of bed days lost by each hospital), broken down across:

  3. Adult patients aged 18-64 years
  4. Child patients aged 0-17 years

 

  1. Please provide the average time (in days) in 2025/26 from being medically fit for discharge to discharge where a CHC-funded complex home care package is required, by NHS Trust or other hospital provider, broken down across:

  4. Child patients aged 0-17 years

  1. NHS Greater Manchester is not responsible for coordinating / liaising discharge from hospital to community health and social care providers. Each hospital has their own discharge /transfer /hub team across Greater Manchester (GM). Therefore, it will be necessary for you to contact each NHS Trust directly for the information you seek.   1. NHS GM follow the national framework for NHS Continuing Healthcare (CHC) and Funded Nursing care, which can be found via this link – National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care – July 2022 (Revised) – corrected May 2023

 

The framework states that ICB’s should not assess individuals for CHC from hospital settings. Therefore, NHS GM does not have the data to respond to this question.
