I write to submit a formal Freedom of Information request under the Freedom of Information Act 2000, submitted simultaneously to various depts of the NHS. Hence please address your department, organisation or knowledge accordingly below.

I acknowledge that each organisation receiving this request can only respond in respect of information held by that organisation. Where your organisation does not hold information relevant to a particular question but is aware that another NHS body or related organisation may hold such information, I respectfully request that you identify that body and provide their Freedom of Information contact details so that I may submit a parallel request directly.

I am submitting this request simultaneously to multiple NHS bodies and related organisations so that the complete picture of relationships, governance and interests or influence can be established across the NHS as a whole.

Unless otherwise specified, all questions in this request relate to the period from GTD Healthcare Limited’s incorporation to the date of this request (Company number 12293359) – that is from the beginning of GTD Healthcare Limited’s existence as an organisation to today or their employees as below.

Section 1 – GTD Healthcare Limited and the Special Allocation Scheme

1. Has GTD Healthcare Limited, its employees, officers, directors or clinical leadership at any time administered, operated, managed or had any involvement in the Special Allocation Scheme (SAS) in Greater Manchester, including but not limited to patient removals, patient placements, case reviews or policy decisions? If so please provide dates, names of individuals involved and the nature of their involvement.

2. Has GTD Healthcare Limited, its employees, officers, directors or clinical leadership at any time administered, operated, managed or had any involvement in the SAS Appeals process in Greater Manchester, including but not limited to sitting on appeal panels, providing administrative support, advising on appeals or influencing appeal outcomes? If so please provide dates, names of individuals involved and the nature of their involvement.

3. Who currently administers the SAS Appeals process in Greater Manchester? Who administered it previously and from what dates? Please provide full details of every organisation that has administered the SAS Appeals process in Greater Manchester from inception to the date of this request.

4. Has Northern Health GPPO at any time had any relationship, contractual or otherwise, with GTD Healthcare Limited? If so please provide details including dates and nature of relationship.

5. Has any GTD Healthcare Limited employee, officer or director at any time sat on, advised or influenced any SAS appeal panel or SAS administrative body that reviewed, adjudicated or made decisions concerning any patient removal submitted by GTD Healthcare Limited itself? If so please provide full details.

Section 2 – GTD Healthcare Limited and NHS Greater Manchester ICB

6. Does any current or former employee, officer, director or clinical leadership member of GTD Healthcare Limited hold or have they held any position, role, seat, membership or influence on NHS Greater Manchester ICB or any of its committees, subcommittees, working groups or advisory bodies? If so please provide names, roles held, dates of appointment and dates of departure where applicable.

7. Specifically – does Dr Brian Lewis, Clinical Chair of GTD Healthcare Limited, hold or has he held any position on NHS Greater Manchester ICB’s Primary Care Commissioning Committee or any other ICB committee or body? If so please provide full details including dates, voting rights and any declarations of interest made.

8. What declarations of interest have been made by any GTD Healthcare Limited employee, officer or director in relation to any NHS Greater Manchester ICB decision, policy, contract or investigation concerning GTD Healthcare Limited or its commissioned services? Please provide copies of all such declarations from inception to date.

9. Has NHS Greater Manchester ICB at any time conducted any investigation, review or audit of GTD Healthcare Limited’s compliance with its contractual obligations, equality duties or patient care standards? If so please provide dates, scope and outcomes of any such investigation, review or audit.

10. What is the total value of NHS contracts held by GTD Healthcare Limited with NHS Greater Manchester ICB from 2020 to the date of this request?

Section 3 –  GTD Healthcare Limited and NHS England / PCSE / Other NHS Bodies

11. Does any current or former employee, officer, director or clinical leadership member of GTD Healthcare Limited hold or have they held any position, role, seat, membership or influence on NHS England or Primary Care Support England (PCSE) or any of their committees, subcommittees, working groups or advisory bodies? If so please provide names, roles, dates and nature of involvement.

12. Has GTD Healthcare Limited or any of its employees, officers or directors at any time had any involvement in the development, administration or review of the PCSE patient removal process or the associated PCSE online portal for immediate removal? If so please provide full details.

13. Does any current or former employee, officer, director or clinical leadership member of GTD Healthcare Limited hold or have they held any position, role, seat, membership or advisory function with the Care Quality Commission (CQC), NHS Improvement, Health Education England, Manchester Health and Care Commissioning (MHCC), or any royal college or clinical advisory body with influence over NHS commissioning, policy or patient allocation decisions? If so please provide full details.

14. Does any current or former employee, officer, director or clinical leadership member of GTD Healthcare Limited hold or have they held any position, role or function with Manchester Health and Care Commissioning (MHCC) specifically in relation to the administration of the Special Allocation Scheme or SAS appeals process? Please provide full details including dates.

15. Has GTD Healthcare Limited or any of its subsidiaries including GTD Academy received any NHS grants, development funding or other financial support from NHS Greater Manchester ICB, NHS England or any other NHS body from 2020 to the date of this request? If so please provide full details.

16. Has any GTD Healthcare Limited employee, officer or director been involved at any stage in any procurement, commissioning or contract renewal process from which GTD Healthcare Limited directly or indirectly benefited? If so please provide full details.

Section 4 – Conflicts of Interest Register

17. Please provide a copy of all entries in your organisation’s conflicts of interest register relating to GTD Healthcare Limited or any of its named employees, officers or directors from GTD Healthcare Limited’s incorporation to the date of this request.

18. Please provide copies of all minutes of NHS Greater Manchester ICB Primary Care Commissioning Committee meetings from 1 January 2024 to date at which GTD Healthcare Limited was discussed or at which any GTD Healthcare Limited representative was present.

19. Has any conflict of interest been declared by any GTD Healthcare Limited representative in relation to any complaint, appeal or investigation concerning GTD Healthcare Limited’s own conduct since incorporation? If so please provide copies of all such declarations.

Section 6 – The Fox Guarding the Henhouse

21. Has any GTD Healthcare Limited employee, officer or director at any time sat on, advised or influenced any panel, committee or body that reviewed, investigated, adjudicated or made decisions concerning a complaint, appeal, investigation or regulatory matter involving GTD Healthcare Limited itself or any of its practices? If so please provide full details.

22. What safeguards exist to prevent GTD Healthcare Limited employees or leadership from having any direct or indirect influence over complaints, appeals or investigations concerning GTD Healthcare Limited’s own conduct?

23. What safeguards exist to prevent conflicts of interest where a commissioned GP provider’s leadership sits on the commissioning body’s committees that oversee that same provider?

24. What process exists for a patient to challenge a decision made by a body where a conflict of interest with their GP provider has not been declared?

Section 7 – Governance and Policy

25. Please provide copies of all policies, procedures and guidance documents held by your organisation relating to the management of conflicts of interest where a commissioned provider’s leadership holds positions on commissioning or oversight bodies.

26. Please provide details of any formal or informal relationship between GTD Healthcare Limited and any NHS complaints handling body, patient advisory body or regulatory oversight body in Greater Manchester from incorporation to date.

Section 1 – GTD Healthcare Limited and the Special Allocation Scheme

  1. GTD Healthcare Limited [12293359] is not contracted to deliver the Special Allocation Scheme.

 

GOTODOC Limited [05401818] is the main trading entity for primary medical services within the GTD Healthcare corporate group. GOTODOC Limited [05401818] provides primary medical services and the Special Allocation Scheme (SAS) from Ashton GP Service, Old Street, Ashton-Under-Lyne, Greater Manchester, OL6 7SR. The Special Allocation Scheme provides primary medical services to those patients that have been immediately removed from their GP practices within the Stockport and Tameside localities.

 

GOTODOC Limited [05401818] was also the provider of Special Allocation Scheme for the Manchester Locality Special Allocation Scheme from 9 January 2023 till 5 May 2025.

 

The SAS provider is commissioned by NHS Greater Manchester ICB to undertake and embed initial appropriate assessment, referring to the commissioner any case where the SAS provider believes the referral may be inappropriate.

 

The SAS provider will also ensure risk assessment and regular monitoring including rehabilitation of the patient’s behaviour which triggered the removal and allocation to the SAS, together with any other behaviours or areas of concern identified through risk assessments and regular monitoring. This is to enable the patient to be repatriated back into mainstream GP practice (primary care) as soon as is reasonable to do so.

 

  1. GOTODOC Limited [05401818] or any provider of the Special Allocation Scheme are not involved in appeals panels.

 

The duties and responsibilities of the ‘Appeal Panel’ will be to objectively review the evidence from the practice that removed and referred the patient to the SAS and the information provided by the patient disputing the referral, to decide whether or not the decision to place the patient on the scheme was appropriate.

 

The membership of the SAS Appeals Panel may consist of:

  • ICB Clinical Lead – Chair
  • Commissioning Lead, ICB
  • Lay Representative

NHS Greater Manchester ICB is made up of 10 localities; each locality manages SAS appeals in line with the requirements as described within NHS England’s, Primary medical services policy and guidance manual (PGM) section 7.5 – Patient appeals process.   1. NHS Greater Manchester ICB is unable to respond to this question, it has no involvement in any external relationship, contractual or otherwise, between providers including GTD Healthcare Limited and Northern Health GPPO.

Section 2 – GTD Healthcare Limited and NHS Greater Manchester ICB

  1. Dr Brian Lewis is a member of the GM GP Provider Board (GM GPB – is a sub-board of the GM Primary Care Provider Board). This is not part of the NHS GM infrastructure. Although GM GPB has a non-voting seat on GM Primary Care Commissioning Committee (PCCC), Dr Lewis has not been the representative at PCCC.

 

Dr Lewis is also the Primary Care Board representative on the Greater Manchester Urgent and Emergency Care Reform Board (GM UEC Board). However, this is a system wide transformation group, with no formal decision-making abilities or delegated duties.

No current or former employee, officer, director or clinical leadership member of GTD Healthcare Limited or GOTODOC Limited hold or have held any position, role or function with Manchester Health and Care Commissioning (MHCC) specifically in relation to the administration of the Special Allocation Scheme or SAS appeals process.

NHS GM has consulted Companies House to identify the subsidiaries of GTD Healthcare Limited; however, this response is provided recognising that there may be subsidiaries of GT Healthcare Limited with NHS GM is not able to identify. However, for all identified companies, there are no specific transactions recorded which meet the distinction of NHS Grants, development funding or other financial support. NHS GM, however, only holds records in respect of NHS Greater Manchester ICB, or the 10 predecessor CCGs (being NHS Bolton CCG, NHS Bury CCG, NHS Heywood, Middleton and Rochdale CCG, NHS Manchester CCG, NHS Oldham CCG, NHS Salford CCG, NHS Stockport CCG, NHS Tameside and Glossop CCG, NHS Trafford CCG and NHS Wigan Borough CCG). The primary care commissioning, procurement and contracting process for NHS GM is very robust around managing potential Conflicts of Interest of this nature.

 

Section 4 – Conflicts of Interest Register

  1. As of 27 April 2026, the conflict-of-interest entries that have been received in relation to GTD Healthcare Limited:
  • Laura Beckett has declared an indirect interest as her husband is employed by GTD Healthcare.
  • Dr Einas Ben Hameida has declared an interest as a general practitioner salaried working with GTD Health care in Lindley Medical Practice in Oldham, as the medical lead in the practice and the Associated Medical Director for GTD and board member with no financial control or gain from GTD. Also board member in OUR PEOPLE TRUST in GTD and LMC member representative with West Pennine LMC.
  • Simone Logue has declared an indirect interest as her husband is the Chief Operating Officer for GTD Healthcare.
  • Samantha McVeigh has declared an interest of outside employment as a clinical assessor for GTD Healthcare provider for 20-40 hours.
  • Dr Manish Prasad has declared an interest as a registered sessional GP GTD healthcare provider.
