I am writing under the Freedom of Information Act 2000 to request the following information in relation to Local Enhanced Services (LES) commissioned from GP practices by your ICB for the financial year 2026/27 (1 April 2026 to 31 March 2027). Where 2026/27 contracts have not yet been finalised or signed, please provide the equivalent information for 2025/26 as the most recently completed year.

  1. A complete list of all Local Enhanced Services (LES), Locally Commissioned Services (LCS), or equivalent locally commissioned GP services currently commissioned from GP practices in 2026/27 (or 2025/26 if 2026/27 is not yet available).
  2. For each service listed, please provide:
  • The service specification, or a link to where it is publicly available
  • The payment rate(s) with the specific £ amount for each payment tier or module — e.g. per appointment, per patient per year, capitation rate per weighted patient, or lump sum
  • The total budget allocated to each service
  • The payment frequency (monthly, quarterly, annually)
  1. Where tariff or payment information has been redacted in any publicly published service specification, please provide the unredacted payment rates.

I am particularly interested in, but not limited to, the following services:

  • Phlebotomy
  • Anticoagulation / INR monitoring (Warfarin)
  • Shared Care / High-risk medicines monitoring (including all banding rates)
  • Wound Care (all tiers including suture removal, simple and complex)
  • ECG (full pathway and interpretation only)
  • Spirometry / FeNO / Respiratory diagnostics
  • Diabetes (annual review and all treatment modules including GLP-1 and insulin)
  • Dementia annual review
  • Palliative / End of Life Care
  • Care Homes (enhanced health in care homes, any top-up to the national DES)
  • PSA monitoring
  • Vaginal Pessary / Ring Pessary for pelvic organ prolapse
  • Intra-uterine system (IUS) insertion and removal
  • Minor Surgery / MSK Injections / Invasive procedures
  • Medicines Optimisation / Prescribing Quality Scheme
  • Learning Disabilities annual health check
  • Ambulance Advice / Support
  • Any other locally commissioned services not listed above

1. Please see attached Beyond Core Contract documents for 26/27 which describes the locally commissioned services with GP Practices for GM provision as well as some local services for individual localities.

2. For each service listed, please provide:

  • The service specification, or a link to where it is publicly available – details within the attached documents.
  • The payment rate(s) with the specific £ amount for each payment tier or module — e.g. per appointment, per patient per year, capitation rate per weighted patient, or lump sum – details within the attached documents.
  • The total budget allocated to each service – each practice is to submit claims accordingly.
  • The payment frequency (monthly, quarterly, annually) – frequency can be monthly or quarterly dependant upon the service – details within the attached documents.

  • Phlebotomy – partially included within the attached document, however, the service is under review during 26/27 as some localities hold differing arrangements.
  • Anticoagulation / INR monitoring (Warfarin) – included within the attached document
  • Shared Care / High-risk medicines monitoring (including all banding rates) – included within the attached document
  • Wound Care (all tiers including suture removal, simple and complex) – not delivered by GP Practices.
  • ECG (full pathway and interpretation only) – delivered by community providers within primary care settings.
  • Spirometry / FeNO / Respiratory diagnostics – commissioned separately to the attached document and are currently under review.
  • Diabetes (annual review and all treatment modules including GLP-1 and insulin) – covered within the attached document
  • Dementia annual review – under review as part of Place / Neighbourhood working
  • Palliative / End of Life Care – some elements are covered under the national
  • Care Homes (enhanced health in care homes, any top-up to the national proactive care for people living in care homes enhanced health in care homes framework DES) – Manchester provide an additional £564 per bed in addition to the DES funding. This was a result of a full-service review under Manchester CCG.
  • PSA monitoring – see table.
  • Vaginal Pessary / Ring Pessary for pelvic organ prolapse – see table.
  • Minor Surgery / MSK Injections / Invasive procedures – see table.

Tameside Ring Pessary An item of service fee of £26.33 will be paid for recorded interventions when carried out within the practice and £52.65 when carried out during a home visit.
Trafford Minor Surgery £44 per injection (£22 if inappropriate referral)

£89 per procedure (£43 if inappropriate referral)

Wigan Ring Pessary £50/patient

 

  • Intra-uterine system (IUS) insertion and removal – commissioned by Local Authorities
  • Medicines Optimisation / Prescribing Quality Scheme – see attached document
  • Learning Disabilities annual health check – LD is no longer within the Quality Outcomes Framework, it’s a Directed Enhanced Services, so we must offer it to all GP practices.
  • Ambulance Advice / Support – not applicable to general practice.
