Under the Freedom of Information Act 2000, I would be grateful if you could provide the following information relating to commissioned ophthalmology services within your Integrated Care Board region.
For the purposes of this request, please exclude primary care General Ophthalmic Services (GOS) routine sight testing and standard optometry services. However, please include commissioned community ophthalmology, ophthalmology, diagnostic, treatment, surgical and referral management services where applicable.
