I am writing to make a FOI request

Please could you provide the contact details (including email addresses and telephone numbers where available) for the following teams within your authority:

1. Adult Commissioning Team responsible for Learning Disabilities, Autism, and Mental Health services
2. Adult Brokerage Team responsible for Learning Disabilities, Autism, and Mental Health referrals/placements
