I am looking for some information to be answered under the Freedom of Information Act relating to neighbourhood health centres.

My questions are as follows:

• When did the ICB submit its proposals for neighbourhood health centres?
• Where do you want your centres to be located?
• How much funding did you request?
• Do your proposals involve constructing a new build centre?
• Do your proposals involve upgrades to existing estate?
• If yes, what estate is this and what is the nature and cost of the upgrade required?
• Do your requests involve the purchase of new land?
• Do your requests involve leasing of a building from a private landlord?
