Please could you provide the following information regarding the independent providers that are currently contracted by your ICB to provide ‘Memory Service’ / ‘Memory Assessment Service’ / ‘Memory Clinic’ / ‘Diagnostic Memory Clinic’ services (or other similarly named services that focus on the assessment of dementia patients).

1. How many independent providers are currently contracted by your ICB to provide ‘Memory Service’ / ‘Memory Assessment Service’ / ‘Memory Clinic’ / ‘Diagnostic Memory Clinic’ services (or other similarly named services that focus on the assessment of dementia patients)?
2. What are the specific names of each of these independent providers and the service each provides?

For the following questions, please provide information for each of the independent providers named in question 2. If the answer to question 1 was zero, please skip these questions. Please give the answer to each question in the format indicated by the table associated with that question.

3. For each independent provider, the total number of referrals received for dementia assessments in each of the calendar years given. Please add rows for additional providers as appropriate.

4. For each independent provider, the overall number of patients that underwent a dementia assessment in each of the calendar years given. Please add rows for additional providers as appropriate.

5. For each independent provider, the number of patients currently on the waiting list for a dementia assessment. Please add rows for additional providers as appropriate.

6. For each independent provider, the average wait time from referral to initial appointment. Please add rows for additional providers as appropriate.

NHS GM ICB contracts NHS Providers only; Greater Manchester Mental Health Trust & Pennine Care Foundation Trust.

There is a service at Bury, however, NHS GM does not hold any of the information requested and will need to be sought from Pennine Care NHSFT. NHS GM ICB has no other independent sector providers of Memory Service / Memory Assessment Service / Memory Clinic / Diagnostic Memory Clinic services (or other similarly named services that focus on the assessment of dementia patients).
