The following response has been provided from our interpretation of the above request. Individual care plans are unable to be shared as they contain patient identifiable data, therefore financial information has been provided for adults eligible for NHS Continuing Healthcare who are placed in a 24 – hour placement and those within their own home.
Q1. 20 highest annual value 24-hour placements for individuals eligible for NHS Continuing Healthcare within GM ICB Bolton Locality.
Ther value of each individual package of care is considered exempt under section 40 (2) of the Freedom of Information Act. This is because the cost of the package of care is specific to the needs of the individual patient. However, NHS GM is able to provide summary information as per the below.
The total Annual Cost of the 20 highest value placements is £3,019,521.
