To remain within the cost limit, I am requesting existing maternity‑dashboard metrics or standard ICS‑level reporting data relating to home‑birth service provision and safety. I am not requesting incident‑level data. 

For each of the past five financial years (or for as many years as your dashboard holds), please provide the following metrics, broken down by year, where they already exist within your ICS dashboard or standard reporting tools:

  1. Number of planned home births
  2. Number of actual home births 
  3. Number of transfers from home to hospital relating to home births
  4. Number of planned home births cancelled, where this metric is already collected within your ICS dashboard or standard reporting tools. If recorded, please also provide the categorised reasons for cancellation.
  5. Number of occasions where home birth services were suspended
  6. Recorded reasons for suspension, as held in your dashboard or reporting tools
  7. Any home‑birth‑related serious incident numbers already included in your ICS dashboard (aggregated only)

If any of the above metrics are not held at ICS level, please confirm this.

Please do not extract incident‑level data – I am requesting only metrics already held in your existing dashboard or reporting systems.
