Description Dynamic Support Register and Complex Placement Commissioning Contacts

For each of the below, please provide the contact for whoever holds commissioning or referral responsibility (i.e. someone able to discuss placement capacity and funding routes with external providers), rather than clinical case-management staff, where these are different roles.

  1. The name, job title, and direct contact details (email and telephone) of the lead commissioner responsible for your Dynamic Support Register.=
  2. The name, job title, and contact details of the lead contact for your Transforming Care pathway or Learning Disability and Autism (LD&A) commissioning team, in relation to adults aged 18+.
  3. The name, job title, and contact details of the lead contact for Continuing Healthcare (CHC) commissioning of complex learning disability/autism placements for adults aged 18+, if this sits with a different team from question 2.
  4. Where your organisation distinguishes between complex and non-complex learning disability/autism cases (for example, separate teams or leads for standard placements versus complex/high-acuity placements), please provide contact details for each, clearly indicating which team covers which caseload.

Where a named individual’s direct contact details cannot be provided, please provide the relevant team’s generic email address and telephone number instead, along with the job title of those who monitor it.

If any of the above sits with a different team, organisation, or Commissioning Support Unit that carries out this function on your behalf, please let me know who I should contact instead.
