Please provide the following information in relation to current individually commissioned packages of care funded wholly or partly by the ICB and delivered in the person’s own home.
Please include packages where the commissioned care/support provision is 20 hours per week or more.
Please exclude care homes, nursing homes and standard domiciliary care commissioned through general homecare contracts.
If any of the information isn’t held or readily available, please provide the remaining information.
As we are transitioning from a locality based model to a centralised NHS GM model, there have been different approaches in each locality and as such, data has been collected in different ways.
