We currently work in several Primary Care Networks across Greater Manchester ICB and are seeking access to recorded information under the Freedom of Information Act 2000 in relation to enhanced services and locally commissioned arrangements delivered through primary care for long-term condition management.

To help us understand the services currently commissioned, please provide the recorded information you hold in relation to the following areas:

1. Enhanced services and incentivisation schemes: Please provide copies of, or links to, any specifications for enhanced services, locally commissioned services or incentive schemes commissioned through primary care for long-term condition management, including diabetes, cardiovascular disease, chronic kidney disease and respiratory conditions. Please also include any prescribing incentive schemes managed through your Medicines Optimisation team.
2. Delivery level: For each service or scheme identified, please confirm whether it is delivered at practice level, PCN level, ICB level or another level.
3. Commissioning method: Please confirm whether each service or scheme is commissioned in bulk, individually, or through another commissioning route.
4. Contracting and payment arrangements: Please provide details of how primary care providers are contracted and paid to deliver each service or scheme, including whether payment is made up front, on completion, by activity, by outcome, or through another payment mechanism.
5. Review schedule: Please confirm when each service or scheme is next due to be reviewed, renewed or recommissioned.
