Please provide the following information relating to GP practices within the Bolton locality of NHS Greater Manchester.

1. Care Home Resident Numbers by GP Practice For each of the last five years for which data are available, please provide:

• Practice code and practice name.
• Total number of registered patients.
• Total number of registered patients resident in care homes.
• Whether each practice achieved the target and, where available, the percentage achievement.
• The payment made to each practice in relation to achievement of the prescribing target.

Please specify the reference date used for these figures in each year.
