• Whether the PCN was deemed to have met the relevant targets.
• The percentage achievement against those targets, where recorded.
• The value of Capacity and Access Improvement funding available to the PCN.
• The amount of funding actually paid to the PCN.
• Any targets, requirements or performance measures relating to online consultations, online access, digital access or online consultation platforms that formed part of Capacity and Access Improvement funding arrangements.
• The funding associated with those requirements.
• Whether the PCN met those requirements.
• The amount of funding paid in relation to those requirements.

Non-Payment or Reduced Payment Decisions

For any PCN that did not receive the full available payment, please provide:

• The reason or reasons recorded for the decision not to make the payment or not to pay the full amount.
• Copies of any assessment criteria, reports, business rules, decision-making frameworks or correspondence used to determine eligibility for payment.
• Any appeal, review or reconsideration process available to PCNs in relation to these decisions.
