I am writing to request information under the Freedom of Information Act 2000 concerning the following GP practice:

Al-Fal Medical Group
Pikes Lane Health Centre
Deane Road
Bolton
BL3 5HP

For the 12-month period 14 October 2025 to 14 October 2026, I would be grateful if you could provide the following information:

1. The names of all individuals recorded as partners or parties to the practice’s NHS primary medical services contract at the beginning and end of this period.
2. Details of any changes to the recorded partnership or contract holders during this period, including individuals joining or leaving.
3. The effective date of each change and, where recorded, the date on which NHS Greater Manchester was formally notified.
4. Copies of any relevant contract variation notices or other records confirming these changes, with unrelated confidential information redacted where necessary.

I am seeking information specifically relating to the identity of the contractual parties and the dates of any changes to their recorded status. I do not require financial information or confidential partnership agreements.

If NHS Greater Manchester does not hold this information, I would appreciate confirmation of which organisation is responsible for maintaining these records.

NHS GM holds this information, however, it is exempt from disclosure under section 43(2) of the Freedom of Information Act 2000.

Section 43(2) of the Freedom of Information Act states:

“Information is exempt information if its disclosure under this Act would, or would likely to, prejudice the commercial interests of any person (including the public authority holding it)”

In this instance, disclosure of this information would be likely to prejudice the commercial interests of NHS GM.
