Children’s Acute Nursing Team

Specialist children’s nurses providing care for children and young people with acute, chronic, and long-term conditions. Available for children under 16 (up to 18 with complex needs or under a paediatrician) living in Manchester and registered with a GP.

Open – 8am–10pm, 365 days a year

Home visits and clinic appointments available. The service aims to prevent unnecessary hospital admissions and support early discharge.

Support includes:

  • Clinical observations and reviews
  • Acute illness and long-term condition support
  • Sleep studies
  • Wound care and suture removal
  • IV/IM/subcutaneous medicationsNGT and gastrostomy care
  • Eczema, tracheostomy, and enteral feeding support
  • Training for families, schools, and carers

Hospital@Home

Same-day visits for conditions including wheeze, bronchiolitis, jaundice, and asthma to avoid hospital admission and support discharge.

Children’s Palliative Care Team

Supports children aged 0–16 with a Manchester GP alongside hospital and community teams.

Open-  Monday–Friday, 8am–4pm

Support includes-

  • Named keyworker support
  • Symptom management and nursing care
  • Rapid discharge and end-of-life support
  • Parent and professional training
  • Sibling support, play therapy, family activities, and memory making

Supports children aged 0–18 with complex needs meeting Continuing Care criteria.

Open: Monday–Friday, 8am–4pm

Support includes-

  • Care package management
  • Family and carer support/training
  • Equipment and consumables
  • Liaison with hospital teams

Children’s Bladder and Bowel Service

Supports Manchester children aged 0–16 with continence and constipation concerns.

Open: Monday–Friday, 8am–4pm

Referrals are accepted for:

  • Daytime/night-time wetting (5+)
  • Chronic constipation
  • Toilet training support for children with learning disabilities
  • Intractable incontinence and continence products

Children’s Asthma Service

Specialist asthma nurses supporting children aged 3–16 (SEND up to 18) with diagnosed or suspected asthma.

Referral criteria include:

  •  Repeated A&E attendance or admission
  • Poor asthma control
  • Frequent reliever inhaler use
  • need for asthma education and inhaler support

Children’s Epilepsy Team

Specialist epilepsy nurses providing support for children and families with a confirmed epilepsy diagnosis.

Open: Monday–Friday, 8am–4pm

Support includes-

  • Home, school, clinic, and telephone reviews
  • Medication and seizure management Education and mental health support
  • Buccal Midazolam training

Children’s Home Enteral Feeding

Specialist dietitians supporting tube-fed children up to 18 years (19 in full-time education).

Open: Monday–Friday, 8am–4pm

Support includes-

  • Community and school clinics
  • Home visits
  • Feeding regimen management
  • Feeding equipment support

Children’s Special Needs Nursing Service

Specialist school nursing team supporting children with severe learning disabilities, autism, and complex medical needs.

Support includes-

  •  School staff training
  • Holiday nursing advice
  • Tracheostomy and gastrostomy support
  • EHCP and care planning support
  • Safeguarding and multi-agency working

Acute Children’s Treatment Service (ACT)

Advanced Nurse Practitioner-led service for acute minor illness.

Open: Monday–Friday, 8am–6pm

Support includes-

  • Assessment, examination, and management plans
  • Referrals when required
  • Referrals accepted from NHS 111, CCHS, GM-CAS, NWAS, and PED.
