The Children’s Community Nursing Team in Rochdale is made up of lots of different health professionals that work across Heywood, Middleton and Rochdale to provide the very best care, support and advice to children, young people, families and carers.
To support easy access, referrals can be made in to the Children’s Community Nursing Team by anyone that works with a child or young person. This includes:
- Any health professional
- Any social care professional
- Any education professional

Northern Care Alliance
The Children’s Community Nursing Team provides specialist nursing care and support for children and young people with acute, long-term and complex health conditions, helping families get care closer to home and reducing unnecessary hospital admissions.
The service is available every day from 8am to 8pm, including bank holidays.
