The Children’s Community Nursing Team (CCN) is provided by the Northern Care alliance to give specialised nursing care for children and young people aged 0-18 with acute, long term and complex health needs working closely with community, hospital services and families to prevent or minimise hospital attendance or admission.
The service runs from 8.00 – 20.00 7 days a week
Referrals to the team are accepted for any child living in Salford from any professional, providing a clinical diagnosis has been made and nursing care is requested.
