The Children’s Community Nursing Team consists of a skill mix of qualified children’s nurses and health care support workers that deliver care to children and young people within a variety of home, community and educational settings.

The Children’s Community Nursing Team provide improved coordination between inpatient and community care. The service will work alongside other healthcare professionals to enable closer inter-professional practice so that families receive integrated / accessible care. The service will provide treatment and continuing management of patients in their own homes or local community, which supports addressing of inequalities in health care.

The service will provide a range of services including nurse led specialist services and clinic based support where necessary, as follows:

  • Acute / Short term Care
  • Chronic / Long Term Conditions
  • Complex and Additional Health Care
  • Palliative and End of Life Care
  • Hospital@Home (previously Virtual Ward)
  • Children’s Continence Product Support

The special educational needs School nursing service consists of a team of qualified children’s nurses that will deliver care and support to children and young people within an educational setting.  The SEN School nursing service will be supported by the generic Children’s Community Nursing Team staff and a specialist Children’s Palliative Care nurse to meet the palliative care needs of the children / young people who attend SEN schools within the Trafford area.

The service will provide improved coordination between services, inpatient care (where appropriate) and community care and will work alongside other healthcare professionals to enable closer inter professional relationships so that families receive integrated / accessible care.

The service will provide a range of services to complex health care needs including:-

  • Provision of a range of training to education staff to enable them to meet the care needs of pupils
  • Facilitate / support attendance at school for pupils with complex health care needs
  • Regular domiciliary and telephone support for parents / carers
  • Attendance and multi-agency meetings
  • Growth monitoring
  • Immunisation programmes
  • Provision of health education
  • Safeguarding children / child protection support
